I have not received my login details for MY PROVADA. Where can I request this?
If you have already received a login code, but lost it, you can request the login code yourself by clicking login on the PROVADA website (top right of the page) and then clicking forgot password in the login screen.
If the system recognizes your email, you will be prompted to create a new password.
PROVADA can and may no longer send you your login details. The passwords are not visible to PROVADA. If you still have questions, you can contact Cerille Roelofsen at cerille@provada.nl
